New Patient Registrations

We are happy to accept any person who lives in Sheffield S8 to register as a patient at this practice. 

If you wish to register with the practice please complete a registration form. These are available via the download below or from the Reception desk at the Practice, please note we are unable to process any incomplete forms. 

Registration forms

Please return the form to the practice by hand, post or email the completed forms to our Practice account: SHECCG.WoodseatsMedicalCentre@nhs.net 

If you are on any repeat medications, if possible, please bring in a copy of your repeat templates so we can re-add these to your records. 

You will be allocated a named accountable GP at the time of registering with the practice. However this does not prevent you from seeing any GP in the practice.  If you are not sure who your allocated named accountable GP is then please ask a receptionist.

Disabled Patients

Our building is fully accessible for disabled patients. We have a limited number of disabled parking spaces in the staff car park located at the back of the Medical Centre. 

Reception is located on the 1st floor of the building, which is accessible via 2 lifts located on the ground floor.
