IMPORTANT NOTICES
Woodseats Medical Centre is committed to keeping the Practice a safe place to protect sick and vulnerable patients & staff from the risk of COVID-19. This means after 19 July patients must continue to wear face masks and socially distance in the Practice, unless medically exempt.



We will be closed from 13:00pm on Tuesday 28th September 2021 for staff training. We apologise for any inconvenience.



MESSAGE FROM DR STAMMERS 

20th September 2021

It is with deep regret that I write to inform you that I will be leaving my position at Woodseats Medical Centre at the end of September 2021. As some of you may be aware, I have been absent from work for a few months and unfortunately the time has come for me to say goodbye.

I came to Woodseats Medical Centre as a newly qualified GP in May 2010 and fell in love with the kind and compassionate culture of the Practice. Serving the people of Woodseats and neighbouring areas has been my absolute honour and privilege. I have been truly humbled by the strength, generosity, and gratitude from many of the wonderful patients and families I have been fortunate enough to know.

All the staff at Woodseats Medical Centre work tirelessly with the aim of providing excellent patient care, which sits at the heart of everything. They will continue to be the best advocate that they can be for you, and I am proud to have been part of this amazing team.

COVID-19 INFORMATION

We are asking patients to avoid any unnecessary contact with the Practice to ensure both patient and staff safety. Do not visit the Practice or A&E if you display any symptoms


Please be aware not to visit the Practice if you answer yes to any of the below questions:

  • Do you have a new cough?
  • Do you have a temperature? (Over 37.8 degrees)
  • Has your sense of taste or smell altered? 
  • Does anyone in your household have any of the above symptoms?

If you answer yes to any of these questions, you may be advised to go home and follow NHS Coronavirus guidance.


Please do not bring visitors to any appointments unless absolutely necessary and only arrive 5 minutes prior to your appointment time where possible.


You may be aware of a new NHS Digital initiative to extract information from GP records for research and other purposes. You may wish to opt out of this data collection and there are 2 different requirements to opt out:

1. Type 1 data opt out - 
This is to protect GP data held on your system. Further infomration can be found here: https://digital.nhs.uk/data-and-information/data-collections-and-data-sets/data-collections/general-practice-data-for-planning-and-research
You need to complete the following form and hand into Reception or email us on sheccg.woodseatsmedicalcentre@nhs.net -  Opt out form

2. Type 2 data opt out - No time limit
This is to protect non-GP data held on your system, eg hospital or clinic treatment.
Patients can do this for themselves by visiting the Make your choice webpage.
For children under 13 the form available here should be used.
For dependant adults the form available here should be used.


 


From 17 May 2021, people in England who have been fully vaccinated against COVID-19 can demonstrate their vaccination status for international travel. A full course is currently two doses of any approved vaccine. Vaccine status will be available from: 

  • the NHS App which you can download from app stores 

It may take more than a week for your identity to be checked and verified so you can use this service.

If you cannot access this online service, and you have had 2 vaccines, you can request a paper letter from the NHS by calling 119. Only call 119 if you are due to travel abroad in the near future and have had your second dose more than 5 working days ago. It may take 7 working days for the letter to arrive.

This practice is not able to provide you with a letter that shows your COVID-19 vaccination status.

 






 



Appointments and general enquiries:

All general enquiries must be made via telephone or email. Our phone lines are operational as usual however during this very busy period, you may experience longer than average wait times. Please note we are unable to advise on whether to self isolate.

Blood tests -

The drive-through blood taking service will be moving location with effect from Saturday 3rd July 2021. It will be moving to City Parkway, Parkway Avenue, Sheffield, S9 4WA from Monday 5th July.

The drive through service at City Parkway will be open Monday to Friday, 7.30am to 5.15pm (excluding Bank Holidays) and Saturday, 8am to 1pm.

Before you attend please ensure your doctor has placed an order on the system or you have a relevant request form. If you do have it please bring your hospital number with you and have this ready for the phlebotomist as this will make it easier to find you on the computer system.

The drive-through service will be operating in addition to the C floor phlebotomy service at the Royal Hallamshire Hospital which is open Monday to Friday, 7.30am – 5.20pm and Saturdays, 8am – 1pm.



NHS app

The NHS app can be downloaded from the app store on most smartphones. You can obtain login details via the app. Please see the following video for more information:
