Patient Participation Group - next meeting Monday 3rd February 2020 at 5pm.

All diabetics need to book into the Diabetic Clinic held on Tuesdays and Thursdays for blood tests, checking blood pressure, height, weight, promotion info and foot checks.  These are 20min appointments

We cannot accept urine samples after 2pm

Flu - Eligible patients can book an appointment for their vaccination with Reception on 0114 2850140.

New Patient Registration

We are happy to accept any person who lives in Sheffield S8 to register as a patient at this practice.  To register, please call at Reception with your Photo ID and proof of residency.

You will be asked to complete a registration form.  You will be given a new patient health information form to fill in so that we have as much of your medical history as possible until your full records arrive from your previous GP.

After registering with the practice you will need to attend the morning drop in clinic (between 08.30 & 10.30 Mon-Fri) for a blood pressure check and to bring a urine sample with you.

If you are on less than 3 repeat medications you will need to speak to a GP via the morning triage service (between 08.30 & 09.30 Mon-Fri) before these can be issued. If you are on 3 or more repeat medications you will need to book an appointment with a GP before these can be issued.

You will be allocated a named accountable GP at the time of registering with the practice. However this does not prevent you from seeing any GP in the practice.  If you are not sure who your allocated named accountable GP is then please ask a receptionist.

Should you leave your registered address and move out of the area, this includes living abroad for more than three months at a time, you may be asked to leave our list.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Accessible Information Needs

Please let us know if you have any information or communication needs.  We will then do our best to meet these needs.  Please inform any member of staff.

Disabled Patients

We operate from an old building which presents some problems around access.  However, we have made the front of the building accessible for wheelchairs.  Disability scooters can park in the drive on the left hand side of the building, providing they do not block access.  Some of the consulting rooms are upstairs.  If you cannot manage the stairs and you wish to see a clinician who works upstairs then please tell us when you book and we will arrange for you to be seen by that clinician downstairs.   We call patients in using a tannoy system.  If you have difficulty hearing then tell us and we will ensure that the clinician is aware of that fact and will collect you from the waiting area.
