Patient Participation Group - next meeting 16th September 2019 at 5pm

From September 9th we will no longer be running the drop in clinic

All diabetics need to book into the Diabetic Clinic held on Tuesdays and Thursdays for blood tests, checking blood pressure, height, weight, promotion info and foot checks.  These are 20min appointments

We cannot accept urine samples after 2pm

We will be holding our first Flu Clinic on Saturday 14th September from 8:30am. This is for Patients aged 65 and Over and is by appointment only. Please call Reception on 0114 2850140 for more details.

Carers Information

If you are looking after someone who has a long term illness, either physical illness or mental illness, you are a carer.

It is important that we know about this so we can help and support you. Your work as a carer is much appreciated and we understand what an enormous strain this can be for you. In order to register as a carer, even if you are not a patient at this practice please download the form at the bottom of this page, complete this and return it to the surgery. Alternatively forms are available in reception.

How can we help you

All of our local pharmacists can deliver prescriptions to your home. If you require this service please inform reception when you order your repeat prescription and we will ensure the prescription is sent to the correct pharmacy.

If you feel you need more help please discuss this with us and we can help you to liaise with social services in order to arrange this. This might be:

  •      more help at home
  •      respite care if you are planning a holiday
  •      regular respite care
  •      adaptions required around the house
  •     nursing care such as dressings.

Young Carers

Do you have children under 18 who you rely on to help look after you?

This happens in lots of families due to:


  • Illness
  • Mental Health problems
  • Physical or Learning Disabilities
  • Alcohol or drug dependency

From 1st April 2015, when the Young Carers (Needs Assessments) Regulations 2015 came into force, all young carers under 18 are entitled to an assessment of their needs.
