We hold various clinics to support patients in their management of their long-term condition (also referred to as chronic diseases). These include Asthma, COPD, Diabetes, CKD, Heart Failure, CVD, Rheumatoid Arthritis, Hypertension and related conditions. These are by appointment only and some are delivered by the Practice Nurses. These appointments last longer than routine 10-minute GP appointments. Please read about our Birthday Medical Reviews for patients with long-term conditions.
We have started to use a Person Centred Approach to Care & Planning for our patients who have a long term condition. We began by working with our diabetic patients and this year we are including patients with COPD. Over time, we are hoping to roll out this approach to all of our patients with a long term condition.
A leaflet with more information about Person Centred Planning is available here .
We will share your care with the hospital staff and the community midwives - reception will be happy to arrange your first appointment with the midwife here at the surgery once your pregnancy is confirmed and the midwife will make the appropriate arrangements for your care.
More information can be found on our Pregnancy Care Planner page.
We provide 8-week postnatal checks at the surgery. At this appointment your baby will receive an assessment by the doctor followed by the Nurse appointment for the baby's first immunisations, and Mum has a full postnatal check-up by the GP. You need to book into this clinic AFTER you have registered your baby as a patient at this practice.
We provide child health surveillance and immunisations according to national and local guidelines.
This is done by a number of different team members:
Our health visitors do a drop-in baby clinic on 2nd and 4th Wednesdays of each month between 12.30 pm and 1.30 pm. Appointments with nurses for immunisations can be made via our reception.
For more information on the Child Health and Immunisation Programme click here
To contact our health visitors phone 0114 305 4351 - this may be an answer phone, leave a message and someone will get back to you.
Patients who are on Warfarin therapy require monitoring by regular blood tests to check their INR levels. This is done by the HCAs and Nurses.
This includes: Contraceptive services, women’s health and STD testing. Please speak to a member of the clinical team if you require these services.
Alongside the GP care for patients with mental health conditions, we have appointments available with a Psychological Wellbeing Practitioner. These are by referral from a GP.
A Specialist Continence Nurse delivers a weekly clinic from the practice.
If you are planning a holiday it is very important that you allow plenty of time for any vaccinations you require. We recommend that you allow at least 8 weeks before your travel date so that we have time to assess your travel needs, book you the required appointments and ensure that you are fully covered BEFORE your travel commences.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
There is further information about countries and vaccinations required on the links below:
Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Please note at this moment in time we are not able to offer complex travel appointments. We also need a minimum of 6 weeks notice to book appointments. Please give us one week to process the form and if you have not heard from a the practice after this week then call our receptionist to see if we are able to make you an appointment.
